Our Health Questionnaire is a Personal Training industry standard that requires us to collect General Information such as date and year of birth, height and weight; Exercise History such as frequency and type of exercise; Family Disease history; Past Medical History; Personal Present Information such as medications and allergies; and Coronary Risk Profile.

If you decide you no longer want to receive email updates from us, the ‘constant contact’ service allows you to ‘opt-out’ at anytime by clicking ‘unsubscribe’ at the bottom of the email. Your email address will then deleted from our contacts database.

3. 3rd Parties. We do not sell your personal information to other vendors or anyone seeking to use your personal information for profit.
